Whether returning home from the hospital, a skilled nursing facility, or a doctor’s visit, we help our patients achieve independence in their community and safely transfer home with confidence. We help make hospital stays shorter and less frequent by providing healthcare in the home.
Our inter-disciplinary team partners with patients, their MD, family, and/or caregivers, to achieve goals and quality outcomes (based on the patient’s diagnosis) that enable patients to more easily perform activities of daily living, manage their condition, and move forward equipped with the knowledge and resources they need in their home.
Who is eligible
Nobilis Home Health Care is Medicare certified and works directly with multiple insurance companies. Most insurances, including Medicare, provide these services at no-cost to the patient or family.
Eligibility for Nobilis Home Health Care services is determined by a patient’s physician.
The charges are based on actual cost of each service. Those fees may be paid directly by the patient or family. Through many private insurance plans, or through Medicare or Medical.
Skilled Nurses
1. Registered nurses (RN’s) providing individual care
and case management for medically prescribed
treatment
2. Licensed Vocational Nurses (LVN’s) working under
a physician’s order and RN supervision administer
medication and treatment and assist the patient
3. When returning to independent activities of daily
living
Physical Therapy
• Assessment of patient’s rehabilitation needs
• Range of motion exercises
• Gait training
• Ultrasound/Heat treatment
Occupational Therapy
• Teach techniques to improve and restore the
level of independence in ADL
• Design, fabricate and fit orthotic and
self-help devices
Speech Therapy
• Communication rehabilitation
(SpeechNoice/Language)
• Swallowing evaluation and rehabilitation
Social Workers
Are available for counseling regarding financial needs,
emotional support, adjustment to illness, and to link
patients and their families to community’s resource.
Supplies & Equipment
Provided as ordered by the physician in coordination
with independent DME suppliers.
